An ADO-based Monitor is configured for a MySQL Database Server and the Monitor State remains "Uninitialized".
This situation may occur if the Monitor is configured and either no Credentials are assigned to be used, or the user name / password supplied is incorrect. When this happens, the driver attempts to prompt the user with a pop-up dialog box to supply a user name and password. Since this occurs and ipMonitor is running as a Service, no user intervention is possible to close the dialog box or provide a different set of Credentials.
Create and assign a Monitoring Credential for the Monitor.

Confirm that the user name and password entered into the specified Credential is correct.
Note: ipMonitor 8 has been tested with MySQL 3.23.54+ and 4.0x Database Servers using MyOLEDB v.3.0 (MyProv.dll 761,856 bytes)
